Once you click the green button above this and opt-in, you will receive an email from my office with a short contact form for you to fill out and return to us. Once we have received your form, one of my staff will contact you to finish setting you up in our system and to answer any questions you have about our program.

We will email you a "Welcome Packet" with a list of commonly ordered tests and prices. You will also get a list of pre-set panels you can order with their respective prices.
- When you are ready to order lab tests for your client, simply fill out the form and email or fax it to us. You will receive a confirmation of your order by email. IT'S THAT EASY!
